General Information

NON NHS SERVICES (PRIVATE) - From 1st January 2010

We are very happy to provide you with medical reports and carry out examinations for:

Private sick notes
Insurance claim forms
Cancellation forms
LGV/PCV medical examinations
Sport examinations
Fitness to attend
Fitness to travel
Private prescriptions
Passport signing
Some vaccination services.

For this and other work which is not provided by the NHS, and is not part of your doctor’s NHS duties a fee is charged.

Fees for these and other additional services are in line with BMA recommended rates as at 1.1.2010 and can be seen here.

Please ask at reception when making the request if you are unsure how much the fee will be.

PAYMENT

Please note that we require payment prior to returning paperwork to you, or the organisation. You may pay with cash or cheque, and the reception staff will be happy to provide a stamped receipt. Cheques should be made payable to “The Oakley & Overton Partnership” please.
