Rural West Primary Care Network – Vacancy for a PCN Care Coordinator for Long Term Conditions (Respiratory)

The Rural West Primary Care Network (PCN) is seeking to recruit a Respiratory Care Coordinator, to work within our PCN’s multidisciplinary healthcare team and enhance our friendly busy network of practices. The Network is made up of two GP Partnerships: Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley, Hampshire.

The main purpose of this role is to support patients with respiratory conditions within Rural West PCN. The successful candidate will play a key role in proactively identifying and working with this cohort of patients, to provide co-ordination and navigation of care and support across health and care services.

The post holder will work closely with the Clinical Leads, GPs and Respiratory Nurses within the Practice teams, making sure that the newly diagnosed patients or those with long term respiratory conditions receive the very best care and review. It will involve ensuring patients have the opportunity for Respiratory reviews and helping them access the support and care they need, supporting them to understand and manage their condition(s) and ensuring their changing needs are addressed. This role is administrative rather than clinical, but the post holder will work very closely with the clinical teams in their delivery of patient services and care. Direct patient contact will be a key element of the role.

The postholder will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve people’s quality of life. They will work alongside our Social Prescribers and Health and Wellbeing Coaches to provide an all encompassing approach to personalised care. They will also work with a diverse range of people from different cultural and social backgrounds, so the ability to work confidently and effectively in a varied and sometimes challenging environment is essential.

Applicants will need excellent interpersonal and communication skills and be organised, energetic, patient and empathetic. The postholder will preferably have experience of working in a primary care setting, ideally in health, social care or other support roles including direct contact with people, families or carers. The post is full time (37.5 hours per week) and requires travel within the Network to the surgeries of both Partnerships, so flexibility of working and access to private transport is essential.
